From time to time we take photos and videos of events happening at Grace Church to use for website and social media publication. For minors, we request a release form, signed by their guardian in order to use their photo. Please download, complete, sign the downloaded PDF form, and send a scanned copy to our Church Administrator, Karen Vernon. 

Expense reimburement FORM

To print a copy of the expense reimbursement form, please download. Once completed and signed by you and the ministry minister, deliver to our Church Administrator, Karen Vernon along with a receipt(s) for the expense(s).

FACILITIES MAINTENANCE REQUEST FORM
